My uncle is a Surgeon , and he is called in when a baby is born with two sets of genitals, both male and female. He is the one that removes one set of plumbing and makes sure the other is installed correctly. He feels that sometimes the wrong one is chosen making for a person who feels that they should be another gender. Note this does not explain all the centuries past , but it does explain some I think of the "friends of Dorothy".
