Ken,
COD = Central Ordnance Depot, ie issued from Bandiana. A 'G52' is an issue form, and the form number for this issue was 178, issued on 30 March 1942. A copy of the form would have wend its way through to the Reverse Lend Lease people for adding the financial amount to the RLL account, and later, once the vehicle was returned, another form would have, in theory, wend its way to the RLL account's people to credit the reduced/used value to the other side of the account ledger.
